Job summary

Erickson Senior Living is seeking a PRN Resident Services Coordinator to provide social work services across Skilled Nursing, Long Term Care, Assisted Living and Memory Care environments. You will perform psychosocial assessments, educate residents and families on advance directives, and coordinate from admission to discharge with the care team.

You will support bereavement needs and participate in resident/family activities, while ensuring compliance with state and federal regulations.

Qualifications

• Minimum of 1 year related experience in long-term care/assisted living preferred with knowledge of geriatric population and dementia.
• Must be available to work through the upcoming holiday season.
• Licensed as a Social Worker per state requirements.
• State-specific regulations may apply; read applicable guidelines.
• Background screening per Florida AHCA Level 2 requirements before start.

Responsibilities

• Completes psychosocial assessments, MDS, and required documentation per policy and regs.
• Educates residents and families about advance directives and assists with completion and management.
• Coordinates social work from admission through discharge and plan transitions with staff.
• Provides psychosocial support to residents and families to aid adjustment.
• Collaborates with mental health providers and provides referrals as needed.
• Develops care plans with input from residents and family; leads care conferences and interdisciplinary meetings.

Join our team as a PRN Resident Services Coordinator to provide social work services to the residents and family members within our Skilled Nursing, Long Term Care, Assisted Living, and Memory Care neighborhoods.

How you will make an impact

• Completes psychosocial assessments, MDS, and other required documentation in accordance with department policy, and State and Federal regulations.
• Educates residents, family, and staff about the importance of advance directives and facilitates the completion and management of such documents when needed.
• Coordinates the Social Work function from admission through discharge to include room changes and transition planning when appropriate in collaboration with staff.
• Provides psychosocial support to residents and families with an emphasis on new residents to ensure successful adjustment to a new environment.
• Collaborates with Mental Health Providers and provides referrals as needed.
• Partners with team members to develop a care plan based on the resident’s preferences, psychosocial, and functional needs; prescribes psychosocial interventions for the team to follow; maintains a care conference schedule; coordinates the interdisciplinary care plan meetings with family members.
• Provides bereavement support to residents, family, and staff as appropriate.
• Participates in functions/meetings relating to resident or family needs in Continuing Care.
• Participates in support group activity and Resident and Family Councils.
• Monitors the psychosocial and medically related social service needs of residents routinely and completes all required documentation in the medical record.
• Collaborates effectively with internal departments and all levels of personnel on a wide variety of resident issues and provides education to staff.
• Maintains working knowledge and ensures compliance with Federal, State, and local regulations regarding long-term care/assisted living/assisted care as well as professional standards and practice.

What you will need

• Minimum of 1 years related experience and/or training in Long-term care/assisted living strongly preferred with knowledge of geriatric population and dementia.
• Must be available to work through the upcoming holiday season.
• Licensed as a Social Work per state requirements.
