Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources; ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsibilities

• Advocate, organize, problem-solve, and deliver results for residents.
• Coordinate programs and activities for residents.
• Serve as liaison to community agencies and service providers; seek out new services for residents.
• Identify low-cost service providers and negotiate discounts.
• Engage residents to identify needs and refer to community services.
• Develop a Resource Directory of state/local providers.
• Sponsor educational events related to health care, agency support, life skills and referrals.
• Provide quality customer service to residents and assist with basic needs.
• Assist residents in accessing services such as meals, transportation, personal services.
• Monitor and relay feedback from residents and families.
• Manage internal and external relationships with strong communication.
