Job summary

Evergreen is seeking a Resident Liaison at Winton Manor in Cleveland to connect residents with community resources and support services. You will act as a bridge between residents, property management, local agencies, and the Owner, fostering well‑being and quality of life.

You will coordinate services, educate residents on available programs, and report on service delivery. Strong communication, organization, and cultural sensitivity are essential for engagement with a diverse resident

Qualifications

• Bachelor's or Associate’s degree in Social Work, Counseling, or another social science field is desired.
• Previous experience as a Social Service Coordinator or Resident Liaison preferred.
• Strong communication and organizational skills.

Responsibilities

• Establish and maintain relationships with local service providers and community agencies to link residents to services.
• Navigate the local service provider network and assist residents in obtaining needed items.
• Coordinate resident activities and prepare a monthly property newsletter.
• Provide quarterly reports on services to the Owner and Director of Social Services, ensuring confidentiality.
• Support case management planning and coordinate discharge planning with hospital staff when needed.
