Job summary

TELACU Residential Management, Inc. in California seeks a Resident Services Coordinator I to provide supportive services to elderly and disabled residents in TRM housing communities.

You will assess needs, educate residents on resources, and advocate for services while coordinating with hospitals, providers, and families. The role involves managing the 504 accommodation process, developing care plans, and supporting grants and reporting, with occasional after-hours work and local travel.

Qualifications

• Bachelor’s degree in Social Work, Gerontology, Psychology or Counseling or equivalent.
• Bilingual English/Spanish preferred
• Ability to advocate, organize and problem-solve.
• Ability to communicate in understandable lay language to residents and staff.

Responsibilities

• Serve as resident advocate when issues affect daily life or well-being.
• Initiate 504 process for reasonable accommodations and manage the process with staff.
• Prepare a Supportive Services Plan to facilitate safe aging in place and maximize program benefits.
• Support and coordinate care, grants, proposals, and reporting as needed.
• Develop resource directories and educate residents about community programs and resources.
• Make referrals to service providers and coordinate with hospital, SNF, rehab facilities and families.
