Job summary

Buckeye Commons Care & Rehab in Kent, OH is seeking an Activities Director to plan and implement a comprehensive program of social, spiritual, educational, and physical activities for residents.

The ideal candidate will hold a Certified Activity Director credential, a Bachelor's degree in recreation therapy, and CNA licensure or willingness to obtain it, plus experience in long-term care and SNF activity programs.

Qualifications

• At least 1 year of prior SNF facility experience.
• Certification as Activity Director and completion of a state-approved training course.
• Bachelor's degree in recreation therapy or related field.
• One-year experience as an Activity Director in long-term care.
• Two years of social/recreational program experience within the past five years, one of which was full-time in a resident activities program in a healthcare setting.
• Currently licensed as a Certified Nurse's Assistant or willing to obtain license.

Responsibilities

• Develop and deliver activity programming to promote residents' opportunities for engagement.
• Complete the Preferences for Routine and Activities section of the MDS, the CAA when triggered, and the Company Recreation/Activity Evaluation within five business days of admission.
• Conduct admissions interviews, documentation, and complete the activities section of the MDS assessment tool.
• Identify activity preferences and current activity pursuits from interviews and clinical records, and document findings.
• Complete standardized assessment data for each resident at regular intervals and develop an individualized plan of care based on the identified needs, strengths, and preferences of the resident.
• Implement strategies to assist residents with increasing their involvement in activities that are appropriate to functional or situational limitations.
• Plan community activities that jointly include the community, staff, families, and residents.
• Assure physician's approval for involvement in activity/recreational programs and maintain precautions/limitations list on each resident.
• Provide residents with opportunities and support for involvement in group and individual leisure pursuits based upon individual interests and needs.
