Job summary

Socket.dev is seeking a dedicated activities coordinator to design and implement programs that meet the physical, mental, and psychosocial needs of residents in accordance with policies and regulations. You will develop Care Plans, coordinate activities across settings, and work with staff to ensure resident participation.

The role requires collaboration with interdisciplinary teams, measurement of outcomes, and maintaining supplies and transportation for activities.

Qualifications

• 1-2 years related experience and/or training or equivalent education required.
• Long term care experience preferred.
• Must have a valid driver's license.

Responsibilities

• Participate in resident Care Plan development.
• Plan activities based on assessments and care plans to reflect residents' needs for stimulation or solace, physical, cognitive and emotional health.
• Plan activities reflecting religious and cultural interests; include night/weekend activities across locations.
• Plan individual, group and bedside activities.
• Maintain a schedule of ongoing activities.
• Coordinate Resident’s Council meetings at least monthly.
• Plan Reality Orientation one-on-one sessions.
• Identify and document activity interests and changes in residents' Care Plans.
• Collaborate with staff to ensure resident participation.
• Coordinate volunteer services and outside resources.
• Maintain supplies for the Activity Department.
• Arrange or provide transportation to activities at other locations.
• Participate in Quality Assurance meetings.
• Compute operating costs, maintain records and prepare management reports.
• Attend Interdisciplinary Team meetings.
