Job summary

Village Health Care in Gresham, Oregon, is seeking a Social Services Assistant to support the Social Services Director in addressing residents’ social, emotional, and psychological needs and plan discharges. Full-time, Monday–Friday, with DOE pay.

Responsibilities include conducting evaluations, arranging social services, assisting with finances, and coordinating with staff while maintaining confidentiality and regulatory compliance.

Qualifications

• Must have a minimum of an Associate’s Degree and meets State certification requirements.
• A minimum of 1 to 2 years working in geriatrics.

Responsibilities

• Meet with residents/families to review policies on advance directives and resident rights.
• Conduct and document social services evaluations identifying problems/needs.
• Provide/arrange social work services as indicated by resident/family needs.
• Assist resident with financial planning as needed.
• Keep families informed of resident changes and issues.
• Collaborate with facility staff on resident issues and discharge planning.
