Job summary

Village Health Care in Gresham, Oregon is seeking a Social Services Assistant to support the Social Services Director in addressing residents’ social, emotional, and psychological needs during their stay. You will assist with discharge planning and coordinating social work services as needed.

The role requires an Associate’s Degree and 1–2 years in geriatrics, with strong written and oral communication skills. Monday–Friday, full-time, with competitive wages and benefits.

Qualifications

• Must have an Associate’s Degree and meets State certification requirements.
• A minimum of 1 to 2 years working in geriatrics.
• Excellent written and oral communication skills.

Responsibilities

• Meet with the resident and family to review policies on advance directives, resident rights and non-discrimination.
• Conduct and document a social services evaluation, including identification of resident problems/needs.
• Provide/arrange for social work services as indicated by resident/family needs.
• Assist resident, as needed, with financial planning.
• Keep the resident’s family informed of resident problems, transfers, and changes of level assignment.
• Meet with the appropriate facility staff concerning resident issues.
• Maintain the confidentiality of all resident care information including protected health information.
• Review complaints and grievances and report actions to the Administrator.
