Job summary

Avante Group's Avante at Ocala Skilled Nursing and Rehabilitation Center is seeking a dedicated Physical Therapist to oversee resident assessments and coordinate care under physician orders. You will diagnose, plan, document, and implement therapy programs, ensuring compliance with state and federal regulations while collaborating with a compassionate team to improve resident outcomes.

Qualifications

• Requires a Doctor/BS in Physical Therapy and appropriate licensure or eligibility.
• Successful completion of the National Certification Examination for Registered Physical Therapist.
• Experience or interest in long-term care settings is preferred.

Responsibilities

• Provide and direct physical therapy services to residents, including assessment and treatment planning.
• Conduct regular screenings and evaluate residents for intervention needs.
• Document treatment plans and progress, and communicate with physicians and families.
• Adhere to confidentiality standards and follow department procedures.
• Support management direction and perform related duties as assigned.
