Job summary

Avante at Orlando Skilled Nursing and Rehabilitation Center is seeking a licensed Physical Therapist to oversee resident assessments, treatment planning, and documentation in our in‑house therapy department. You will work with a compassionate team, perform screenings, initiate referrals, and ensure compliance with AHCA standards.

A BS in Physical Therapy, successful completion of the National Certification Examination for Registered Physical Therapists, and active state licensure or eligibility

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• 0–3 years of long-term care experience.

Responsibilities

• Provide Physical Therapy services to residents and initiate referrals as appropriate.
• Screen residents at regular intervals to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Adhere to confidentiality standards.
• Implement and conduct treatments per plan.
• Follow management direction.
• Perform other duties as assigned.
