Job summary

Cura Health is seeking a compassionate Social Worker to join our interdisciplinary care team in providing resident-centered psychosocial support. You will assess needs, support families, and help with care planning and transitions to ensure quality of life for our residents.

Ideal candidates display strong communication and problem-solving skills, maintain confidentiality, and collaborate effectively with nursing and other disciplines to advocate for resident rights and preferences.

Qualifications

• Strong communication, interpersonal, organizational and problem-solving skills.
• Ability to navigate sensitive situations professionally.
• Passion for supporting residents and families.

Responsibilities

• Complete psychosocial assessments and documentation for residents per facility, state, and federal requirements.
• Identify residents' psychosocial, emotional, behavioral, and social needs and develop interventions.
• Participate in interdisciplinary care planning and advocate for resident preferences and dignity.
• Provide support, education, and resources to residents and families during admission, stay, discharge, and transitions of care.
• Coordinate discharge planning and connect residents and families with services and placements.
• Assist with admissions and transitions to promote a positive resident and family experience.
• Maintain confidentiality and ensure HIPAA compliance.
