Job summary

Complete Care in Holmdel Township, NJ is seeking a dedicated Social Worker to support residents and families with psychosocial needs. You will complete Social Histories, assessment, and collaborate with an interdisciplinary team to safeguard residents' rights and wellbeing.

The role requires an LSW or CSW credential; long-term care experience is preferred but not mandatory. You will operate in a compassionate setting and contribute to a supportive care environment.

Qualifications

• Must have LSW or CSW.
• Long-term care experience preferred but not required.
• Ability to work independently or as part of a group.
• Computer knowledge.

Responsibilities

• Identify residents' psychosocial, mental, and emotional needs.
• Complete Social History and Psychosocial assessment of residents.
• Work with interdisciplinary team to promote and protect residents' rights.
• Prevent and address resident abuse as mandated by law and licensure.
• Other duties as assigned by Director of Social Services and Administrator.
• Ability to build and maintain rapport with residents and peers.
