Qualifications

• Bachelor's degree in Social Work or related field required; Master's degree preferred.
• Experience with older adults in skilled nursing or senior living settings.
• Strong communication, interpersonal, and organizational skills.
• Proficiency with electronic documentation and Microsoft Office.

Responsibilities

• Advocate for residents' psychosocial, emotional, and social needs.
• Complete social services assessments and care plan reviews.
• Meet with residents and families to address concerns and support problem-solving.
• Coordinate care conferences and interdisciplinary team meetings.
• Assist with discharge planning and transition to/from Skilled Nursing.
• Maintain confidential documentation per HIPAA and facility policies.
