Job summary

Bear Mountain Health and Rehabilitation in Asheville, NC is seeking a Social Services Director to lead the social services program, ensure regulatory compliance, and coordinate with the interdisciplinary team to support residents and families.

Qualifications

• Bachelor’s degree in social work or related field.
• Minimum one year supervised social work experience in healthcare.
• English communication skills, both verbal and written.
• Highest level of professionalism with confidentiality.

Responsibilities

• Complete the social services portion of the MDS, assessments, and care plans per regulations.
• Assist residents and families with social, emotional, and family issues.
• Participate in resident care planning to identify social and emotional needs.
• Maintain progress notes and ensure chart documentation per policy.
• Refer to support agencies as needed by the interdisciplinary team.
• Prepare discharge plans and transition for residents.
• Serve as information source for families, residents, and the public.
• Coordinate Family Council Meetings and resident room changes.
• Engage families and residents in planning and understanding resident status.
