Qualifications

• Bachelor's degree in Social Work or related Human Services field (e.g., Sociology, Gerontology, Special Education, Rehabilitation Counseling or Psychology).
• For buildings with 120 beds or less: Bachelor's degree required.

Responsibilities

• Coordinates and directs Social Services in accordance with regulations and guidelines.
• Demonstrates rights, dignity and individuality of residents in all interactions.
• Advocates for residents' rights and psychosocial well-being.
• Completes psychosocial assessments and provides treatment recommendations; refers to appropriate mental/behavioral health providers.
• Coordinates discharge plans with residents and families.
• Assists with advance directives in accordance with Federal regulations.
• Resolves and tracks grievances.
• Coordinates family council meetings.
• Coordinates ancillary appointments as necessary.
