Spring View Nursing and Rehabilitation is seeking a Social Services Director for our skilled nursing facility. The role focuses on advocating for residents, coordinating with families, and leading psychosocial care planning to ensure dignity and comfort.

The Director collaborates with a multidisciplinary team to provide emotional support, coordinate resources, and ensure compliance with healthcare social services regulations in a resident-centered environment.

Qualifications

• Bachelor's degree in Social Work or related field.
• One year of supervised social work experience in a health care setting.
• Strong communication and interpersonal skills to interact with residents, families, and staff effectively.
• Empathetic and caring nature, with a genuine desire to improve the lives of our residents.
• Knowledge of relevant regulations and laws related to social services in a healthcare environment.

Responsibilities

• Conduct comprehensive assessments of residents' psychosocial needs and develop personalized care plans in collaboration with the interdisciplinary team.
• Provide emotional support, counseling, and resources to residents and their families to address social and emotional well-being.
• Assist residents in accessing community resources and services to meet individual needs and preferences.
• Facilitate family meetings and serve as a liaison between residents, families, and the facility to ensure open communication and understanding.
• Collaborate with medical professionals to ensure holistic care of each resident.
• Assist residents with discharge planning and coordinate transitions to home or other care settings when necessary.
• Maintain accurate and confidential resident records, ensuring compliance with regulations and policies.
