Job summary

Heritage of Emerson is seeking a compassionate Physical Therapist to join our rehabilitation team. You will assess, develop, and implement individualized plans to help residents maximize mobility and independence within a resident-centered, interdisciplinary environment.

Qualifications

• Current Physical Therapist license in Nebraska.
• Strong clinical assessment and communication skills.
• Commitment to resident-centered care.
• Ability to work in a collaborative team environment.
• Experience in skilled nursing, rehabilitation, or long-term care preferred but not required.

Responsibilities

• Evaluate residents and patients to determine therapy needs and goals.
• Develop and implement individualized treatment plans.
• Provide interventions to improve strength, balance, mobility, and independence.
• Collaborate with nurses, caregivers, and rehab team members for coordinated care.
• Educate residents and families regarding treatment plans, progress, and wellness strategies.
• Maintain accurate documentation per professional and regulatory standards.
• Foster positive relationships with residents, families, and team members.
