Sunnyvale Gardens Post-Acute seeks a dedicated Physical Therapist (PT) to join our rehabilitation team full-time in Sunnyvale, CA. You will assess residents, design personalized therapy plans, and guide them toward maximum mobility in a collaborative, patient-centered environment.

Qualifications

• Master's or Doctorate in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist license in California.
• Current CPR/BLS certification.
• Experience in a SNF/post-acute/outpatient rehab preferred, new grads welcome.

Responsibilities

• Conduct comprehensive initial evaluations of residents to determine physical condition and rehab potential.
• Develop and modify individualized plans of care based on resident needs and physician orders.
• Provide evidence-based PT treatments including strength, balance, and mobility training.
• Document resident progress and discharge plans in compliance with guidelines.
• Educate residents and families on post-discharge exercises and safety protocols.
• Collaborate with interdisciplinary care team to optimize outcomes.
