Job summary

Rosemanorrehab is seeking a licensed Physical Therapist (PT) to provide skilled therapy to residents in a collaborative healthcare setting. You will work with residents, families, nursing staff, and interdisciplinary teams to support recovery and quality of life.

The role emphasizes clinical excellence, documentation, and resident-centered care, with Full-Time, Part-Time, or PRN options and a focus on mobility, function, and independence.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, active Physical Therapist license in the state of practice.
• Experience in long-term care or skilled nursing preferred.
• Strong clinical documentation and communication skills.
• Patient-focused, compassionate and team-oriented.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and care plans.
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely.
• Collaborate with nursing and interdisciplinary teams to coordinate resident care.
• Participate in care planning and utilization review meetings.
• Educate residents, families, and staff regarding therapy goals, recommendations, and plans of care.
• Maintain established productivity and documentation standards.
• Support regulatory compliance and survey readiness.
• Promote resident safety, independence, and quality of life throughout the rehabilitation process.
