Qualifications

• Education: High School graduate or equivalent.
• Education Preferred: B.S. in Recreation Therapy.
• Minimum Experience: Two years' experience as an Activity Coordinator in a long term care setting preferred.
• Certification Required: California Certified Activity Director or Certified Recreation Therapist.

Responsibilities

• Performs comprehensive assessment for each resident. Documents response to care plans and evaluates activity progress.
• Utilizes programs including music therapy, pet therapy and aroma therapy to reduce anxiety, depression, aggression and pain.
• Supports residents through one-on-one, small group and large group activities; records Resident Council interactions and follows up on concerns.
• Supervises activity assistants and develops community volunteer efforts.
• Assists residents to and from activities and arranges off-site trips; orients residents to facility and ensures transportation for activities.
• Collaborates with rehabilitation to promote functional independence and with social services for psychosocial interventions; coordinates with Interdisciplinary Team on behavioral concerns.
• Maintains equipment and supplies within budget; develops meaningful activity programs for all functioning levels.
• Develops schedules and monthly activity calendars covering all residents, including those unable to leave rooms.
• Acts as a member of the IDT team and conducts initial and periodic activity assessments for residents.
• Maintains resident activity plans, progress notes, attendance records and materials used in programs.
• Keeps a conspicuous calendar of activities and records interactions with residents.
