• Bachelor's degree in Social Work or a related field required.
• Experience in senior living, healthcare, case management, social services, or care coordination preferred.

Responsibilities

• Build relationships with residents and families.
• Assess social, wellness, and support needs of residents.
• Coordinate transitions across care continuum (Independent Living to Skilled Nursing).
• Connect residents with internal and external resources.
• Advocate for residents as liaison among residents, families, physicians, and departments.
• Monitor resident wellness and identify opportunities for support.
• Coordinate health education programs, support groups, and wellness initiatives.
• Participate in interdisciplinary meetings to promote resident-centered care.
