Qualifications

• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Build meaningful relationships with residents and their families.
• Assess residents' social, wellness, and support needs.
• Coordinate transitions across Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Rehabilitation.
• Connect residents with internal and external community resources.
• Advocate for residents while liaising with residents, families, physicians, and departments.
• Monitor resident wellness and identify opportunities for additional support.
• Coordinate health education programs, support groups, and wellness initiatives.
• Participate in interdisciplinary meetings to promote resident-centered care.
• Assist with welcoming new residents through orientation and ongoing support.
• Promote resident engagement and well-being through collaboration and compassionate service.
