Qualifications

• Current Social Work license in Alabama – REQUIRED.
• Degree in Social Work from an accredited program.
• Skilled nursing, rehabilitation, long-term care, hospital, or healthcare social work experience preferred.
• Knowledge of discharge planning and community resources preferred.
• Strong communication, organization, problem-solving, and interpersonal skills.
• Compassionate and committed to resident-centered care.

Responsibilities

• Assess residents' psychosocial needs and develop appropriate care plans.
• Provide support and resources to residents and families.
• Participate in care plan meetings and interdisciplinary team collaboration.
• Assist with discharge planning and coordinate community services and resources.
• Support residents with adjustment, emotional, social, and family concerns.
• Maintain accurate and timely documentation.
• Assist with resident rights, grievances, advance directives, and regulatory requirements.
