Park Manor Health and Rehabilitation LLC is seeking a compassionate Licensed Social Worker to join our interdisciplinary care team in Northport, AL. This full-time role focuses on coordinating services, addressing psychosocial needs, and planning for safe transitions of care.

The Social Worker will assess residents' psychosocial needs, develop care plans, support families, and participate in discharge planning and team meetings. Strong communication and compassionate care are essential.

Qualifications

• Must hold current Alabama Social Work license (required).
• Degree in Social Work from an accredited program.
• Experience in skilled nursing, rehabilitation, long-term care, hospital, or healthcare social work preferred.
• Knowledge of discharge planning and community resources preferred.
• Strong communication, organization, problem-solving and interpersonal skills.

Park Manor Health &rehabilitation is seeking a compassionate and organized Licensed Social Worker to join our interdisciplinary care team.

The Social Worker plays an important role in supporting residents and families throughout their stay, helping coordinate services, address psychosocial needs, and plan for safe and successful transitions of care.
