Job summary

Rockport Healthcare Services in Oakland, CA is seeking a qualified Resident Services Coordinator to identify and address residents' psychosocial and concrete needs in line with regulations and company standards.

You will orient residents, coordinate discharge planning, maintain care plans and history, and communicate with families and staff. The role requires strong organizational skills, attention to detail, and collaboration with consultants and the QMPI program.

Qualifications

• High School Diploma or equivalent required.
• Neat appearance and good health are expected.
• Strong written and verbal communication skills.
• Strong interpersonal and organizational abilities.
• Knowledge of social resources in the community is important.
• Degree in Psycho/Social Health is preferred.

Responsibilities

• Ensure residents' psychosocial and concrete needs are identified and met per requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents' financial affairs.
• Communicate needs and plan of care to resident, families, staff.
• Participate in meetings, in-services and training.
• Meet with consultants as required.
• Organize and facilitate Family Council (as requested).
• Implement and maintain QMPI program.
• Assist with coordination of resident room moves.
