Job summary

Rockport Healthcare Services seeks a Social Service Coordinator to address residents' psychosocial and practical needs within a center. You will coordinate discharge planning, care plans, and family communications while ensuring compliance with regulations.

You will maintain records, support resident financial coordination, and contribute to continuous quality improvement through QMPI activities. Strong communication and organizational skills are essential for success in this role.

Qualifications

• Bachelor's Degree in Social Work or human services and one year of supervised health care experience.
• LSW in TX Centers is required where applicable.
• High School Diploma or equivalent for centers under 120 beds.
• Degree in Psycho/Social Health area preferred.

Responsibilities

• Ensure residents' psychosocial and concrete needs are identified and met per federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents' financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and staff.
• Participate in meetings, trainings and required in-services.
• Meet with consultants as required.
• Organize and facilitate Family Council meetings as requested.
• Implement and maintain QMPI program.
• Assist with coordination of resident room moves.
• Present professional image and adhere to company standards in consumer interactions.
• Organize and maintain a professional work setting.

Education

Bachelor's Degree in Social Work or human services

LSW in TX Centers

High School Diploma or equivalent
