Job summary

Southern HealthCare Management in Boynton Beach, Florida, is seeking a dedicated candidate to support the social service function in a long-term care nursing center. You will work closely with the Social Services Manager to ensure positive experiences for new residents, completing necessary assessments and care plans.

Qualifications

• High School Diploma or equivalent required.
• Post-secondary education or work experience in a social service or health related field is a plus.
• Must be familiar with Medical Terminology.
• Must be proficient in the use of personal computers.

Responsibilities

• Support the social service function in a long-term care and short-term rehabilitation nursing center.
• Work with the Social Services Manager to admit new residents and document assessments and care plans.
