Job summary

Conviva Senior Primary Care in Boynton Beach, FL is seeking a Care Coach to provide proactive care coordination and social support for high‑risk patients. You will serve as the primary contact, coordinate with primary care, specialists and community resources, and reinforce care plans to improve health outcomes.

The role involves travel to patients’ homes and clinics, Monday–Friday hours, and a 40‑hour work week with potential overtime. A TB screen and driver requirements are part of the role.

Qualifications

• 3+ years in ambulatory, primary care, or senior-care with direct patient contact.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfort with home visits and community outreach.
• Experience in care coordination and social support for high‑risk or geriatric populations.

Responsibilities

• Conduct structured patient interviews and collect health information.
• Perform home visits to assess living conditions and social factors.
• Identify barriers to care and connect patients with community resources.
• Educate patients on chronic disease management using approved materials.
• Coordinate care across primary care, specialists, pharmacies and home health.
