Job summary

Alhambra Post Acute is seeking a qualified Social Worker to support residents and families, assist in care planning, provide emotional support, and coordinate resources with the interdisciplinary team.

Required is a Bachelor's Degree in Social Work or Human Services plus 2 years of supervised social work experience in a health care setting; MSW preferred. Strong communication, documentation, and a patient-centered approach are essential.

Qualifications

• Bachelor's Degree in Social Work or Human Services with related supervised experience in health care.

Responsibilities

• Establish course of action by exploring options with resident and families.
• Assist residents in achieving highest practicable level of self-care, independence and well-being.
• Provide medically related social services to attain or maintain highest practicable physical, mental and psychosocial wellbeing.
• Evaluate social and family information to develop care plans for social services issues.
• Provide emotional support and address emotional problems including coping with disabilities, health fears, and loss.
• Assist residents with health care decisions.
• Assist with personal shopping.
• Assist in inventory and tracking patient belongings.
• Coordinate response to missing, lost or stolen belongings.
• Assist in obtaining resources from community and social services and health agencies.
• Assist in securing prosthetics and assistive devices.
• Develop and maintain relationships with other departments and community agencies to meet residents' needs.
• Document resident social service status.
• Assist in arranging outpatient appointments and on-site ancillary services.
• Assist in discharge planning with appropriate agencies and personnel.
• Provide in-service training to staff.
• Counsel residents and family members.

Education

Bachelor's Degree in Social Work or Human Services
