Job summary

ALIYA of Glenwood is seeking a dedicated Social Services Director to lead our Social Services Department and support the well-being of our residents. You will work closely with residents, families, facility leadership, and the interdisciplinary team to ensure residents receive appropriate support, resources, and advocacy.

Responsibilities include program development, psychosocial assessments, care-plan collaboration, counseling, and ensuring documentation and regulatory compliance.

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Minimum 2 years of experience in social work or case management within a skilled nursing facility is required.
• Previous supervisory or leadership experience is preferred.

Responsibilities

• Develop, implement, and oversee social services programs to meet residents and families’ needs.
• Complete psychosocial assessments and identify social, emotional, and psychological needs.
• Collaborate with interdisciplinary teams to create resident-centered care plans.
• Provide counseling, advocacy, and appropriate interventions to residents and families.
• Lead and supervise social services staff and provide training and support.
• Assist with adjustment to the facility and changes in health or independence.
• Coordinate referrals to community resources and outside agencies.
• Support discharge planning and safe transitions of care.
• Develop relationships with community organizations and service providers.
• Ensure accurate documentation of social services activities and resident progress.
• Monitor department performance and pursue improvements.
• Ensure compliance with federal and state regulations and facility policies.

The Social Services Director will work closely with residents, families, facility leadership, and the interdisciplinary team to ensure residents receive appropriate support, resources, and advocacy throughout their stay.

Qualifications

• Bachelor’s degree in Social Work or a related field required.
