Job summary

Smith County Health and Rehabilitation seeks a dedicated Social Services Director to lead resident support and coordinate psychosocial care within our long‑term care setting. You will work with an interdisciplinary team to assess needs, develop care plans, and advocate for residents and families.

This role requires a Bachelor’s in Social Work, strong communication, empathy, and a commitment to regulatory compliance.

Qualifications

Responsibilities

• Conduct assessments of residents' psychosocial needs and develop personalized care plans with the interdisciplinary team.
• Provide emotional support, counseling, and resources to residents and families to address social and emotional well-being.
• Assist residents in accessing community resources and services.
• Facilitate family meetings and liaise between residents, families, and the facility to ensure open communication.
• Collaborate with medical professionals to ensure holistic care.
• Assist residents with discharge planning and transitions to home or other care settings.
• Maintain accurate and confidential resident records in compliance with regulations.
• Stay updated with changes in regulations and best practices related to social services.
