Job summary

CommuniCare Health Services in Greenwood, IN seeks a Resident Services Director to plan, assess, coordinate, and implement services that enhance residents' social and psychosocial wellbeing while ensuring high quality care.

You will lead activities and social services, advocate for residents, document findings, and collaborate with staff, residents, and families to achieve outstanding customer satisfaction.

Qualifications

• 4 year college degree in Social Work or related field.
• 1 year of prior work/life experiences, preferably in a healthcare setting.
• Must be licensed by the State Licensure exam through the State Social Worker and Counselor Board.

Responsibilities

• Develop and maintain a good working rapport with direct care givers to support residents’ psychosocial wellbeing.
• Lead Resident Council Monthly meetings and keep records.
• Lead Family Council Monthly meetings and keep records.
• Create and provide monthly Support Group meetings.
• Monitor the living space of the facility and address issues with staff action as needed.
• Participate in care conferences and center policymaking affecting resident care planning.
• Advocate for residents’ rights and involve family members, staff and residents.
• Maintain confidentiality of resident/family information and report grievances to the Administrator.

Education

Bachelor's degree in Social Work or related field
