Qualifications

• Bachelor's degree in social work or human services; LSW in TX centers may be required.
• High school diploma or equivalent.
• Neat appearance and professional presentation.
• Good physical and mental health.
• Strong written and verbal communication skills.
• Solid interpersonal and teamwork abilities.
• Administrative and organizational strengths.
• Knowledge of social resources in the community.
• Degree in Psycho/Social Health area preferred.

Responsabilités

• Ensure residents' psychosocial and concrete needs are identified and met per requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referrals.
• Assist in coordinating residents' financial affairs.
• Communicate needs and plan of care to resident, families, staff.
• Participate in meetings, trainings, and required in-services.
• Meet with consultants as required.
• Organize and facilitate Family Council meetings.
• Contribute to QMPI quality improvement initiatives.
• Assist with coordination of resident room moves.

Formation

Bachelor's degree in Social Work or human services field

High School Diploma
