Job summary

Rockport Healthcare Services seeks a Medicare MDS Coordinator to oversee the resident assessment process and coordinate care through the interdisciplinary team. The role emphasizes timely MDS completion and accurate documentation within regulatory standards.

You will implement and evaluate care plans, administer nursing interventions as ordered, and maintain infection control and safety practices while communicating professionally with residents and families.

Qualifications

• Current licensure in nursing (LVN) required.
• Written and verbal English communications required.
• Strong administrative and organizational abilities.
• CPR certification preferred.
• Two years of long-term care nursing experience preferred.
• Supervisory experience preferred.

Responsibilities

• Coordinate the Medicare/MDS resident assessment process.
• Ensure timely MDS assessments by the Interdisciplinary Team.
• Coordinate development, implementation and evaluation of plan of care.
• Provide treatments and nursing interventions as per plan or physician orders.
• Provide nursing care per infection control standards.
• Follow safety policies in nursing care and emergency measures per policy.
• Maintain essential information for quality resident care and compliance.
• Ensure documentation meets federal/state regulations and policy.
• Participate in meetings and inservices; uphold professional image with residents.
