Job summary

Novato Healthcare Center is seeking a Social Service Coordinator to ensure residents' psychosocial and practical needs are identified and addressed in compliance with federal, state, and company requirements. The role requires a degree in social work or a related field and relevant healthcare experience.

You will orient new residents, participate in discharge planning, maintain care plans, and coordinate financial matters with families while collaborating with staff.

Qualifications

• A Bachelor's degree in Social Work or a human services field and one year supervised healthcare experience.
• High School Diploma or equivalent for centers under 120 beds.
• Good written and verbal communication skills; strong interpersonal abilities.

Responsibilities

• Ensure residents' psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents' financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required.
• Organize and Facilitate Family Council (Participate as requested by Council Members).
• Implement and maintain an ongoing Quality Measurement And Performance Improvement Program (QMPI)
• Assist with coordination of resident room moves.
• Present professional image to consumers through dress, behavior and speech.
• Adheres to company standards for resolving consumer concerns.
• Ensures that all consumer/resident rights are protected.
