The position includes coordinating admissions with the Executive Director, supervising move-ins, addressing family and resident concerns, coordinating care with home health agencies, arranging transportation, and supervising

Responsibilities

• Caregiver scheduling and resident assignments, working within department allowances
• Coordinate admissions with Executive Director including supervising move-ins to be sure accommodations are as desired and care is immediately implemented
• Family/resident admission interviews
• Immediately bring prohibited conditions or at-risk residents in need of a higher level of care to the attention of the Executive Director
• Coordinate care planning with home health agencies on site, working within community policy
• Arrange for transportation as desired by the resident with the Transportation staff
• Arrange for resident special needs involving other departments, verifying follow through
• Function as a liaison with families ensuring special needs/requests/complaints are addressed
• Inform Executive Director, in writing and verbally, of all family or resident complaints
• Monitor staff performance, providing or arranging assistance as needed
• Read all communication notes regarding the community between the Caregiver shifts
• Coordinate staff training and in-service schedules with the Executive Director
• Supervise the medication room and orders, working with and supervising Medication Aides and Caregivers
• Other duties as assigned
