Job summary

12 Oaks Management in Denver, CO is seeking a dedicated Resident Care Manager to oversee resident health assessments and supervise medication administration. This full-time role coordinates care with physicians, home health agencies, and families to ensure high-quality, compliant care.

The ideal candidate holds a nursing diploma, active RN/LPN/LVN licensure in the state, and at least two years of relevant experience with certification, and will participate in staff training, safety practices,

Qualifications

  • Diploma in Nursing is the minimum education requirement.
  • Current nursing licensure (RN/LPN/LVN) issued by the state licensing agency.
  • At least two years of relevant experience is required.
  • One certification from an accredited organization or actively pursuing certification.

Responsibilities

  • Provides care following established nursing service objectives and standards.
  • Ensures that all safety and infection control practices are followed.
  • Distributes and documents medications (including narcotics) according to established standards and regulations.
  • Responds to emergency situations.
  • Ensures that required documentation is complete and is in compliance with regulations and standards.
  • Communicates with resident and families regarding treatment and care.
  • Attends all staff meetings, trainings, and educational classes as required.
  • Performs other duties as assigned.

Education

The Resident Care Manager performs resident health assessments; provides training/oversight to staff for medication supervision, monitors the status of resident general conditions, as allowed by regulation; coordinates resident care with third-party providers such as physicians and home health agencies and companion services in conjunction with the family.

Primary Duties & Responsibilities:
  • Provides care following established nursing service objectives and standards
  • Ensures that all safety and infection control practices are followed
  • Distributes and documents medications (including narcotics) according to established standards and regulations
  • Responds to emergency situations
  • Ensures that required documentation is complete and is in compliance with regulations and standards
  • Communicates with resident and families regarding treatment and care

  • Performs other duties as assigned
