Job summary

Salem Transitional Care is seeking an RN Resident Care Manager to lead resident care planning and coordinate clinical services in a fast-paced long‑term care setting.

You will conduct assessments, develop individualized care plans, monitor resident conditions, and collaboratively work with physicians, families, and interdisciplinary teams to ensure high‑quality, person‑centered care.

Qualifications

  • Current RN license in Oregon
  • Strong clinical assessment and critical thinking skills
  • Excellent communication, leadership, and organizational abilities
  • Compassionate and resident centered approach to care
  • Six months of long term care or skilled nursing experience preferred
  • Supervisory or leadership experience preferred

Responsibilities

  • Oversee and coordinate resident care planning and clinical services
  • Conduct resident assessments and assist in developing individualized care plans
  • Monitor resident conditions and communicate changes with physicians and team members
  • Review physician orders, test results, and treatment plans to ensure continuity of care
  • Utilize electronic medical records to document and maintain accurate clinical information
  • Communicate with residents and families regarding care plans and updates
  • Participate in clinical meetings and collaborate with nursing leadership and other departments
  • Provide education and support to residents, families, and staff regarding care needs
  • Support admissions, transitions of care, and discharge planning processes
  • Ensure clinical documentation is accurate, complete, and compliant with regulations
  • Support and guide nursing staff to promote a positive team environment
  • Use clinical judgment to provide safe, high-quality resident care
  • Follow all regulations and nursing standards
  • Maintain a resident-focused culture throughout the facility

Join our nursing leadership team as an RN Resident Care Manager and earn up to $8,000 in additional bonus compensation throughout your first year with us.

The bonus is paid as you reach the following milestones:

  • 90 Days: $1,500
  • 6 Months: $1,500
  • 9 Months: $2,000
  • 12 Months: $3,000

Total First-Year Bonus Opportunity: $8,000

  • Follow all federal, state, facility, and company regulations and nursing standards
  • Help maintain a positive, supportive, and resident focused culture throughout the facility
What We’re Looking For
  • Current RN license in the State of Oregon
  • Valid CPR certification
  • Strong clinical assessment and critical thinking skills
  • Excellent communication, leadership, and organizational abilities
  • Compassionate and resident centered approach to care
  • Ability to multitask and stay organized in a fast paced environment
  • Strong documentation and care coordination skills
  • Ability to work collaboratively with residents, families, providers, and interdisciplinary teams
  • Dependable attendance and professionalism
  • Long term care or skilled nursing experience preferred
  • Supervisory or leadership experience preferred

Requirements

Education
  • Graduate of an accredited School of Nursing
  • BSN preferred
Licenses/Certifications
  • Current and active RN license in the State of Oregon
  • Valid CPR certification
Experience
  • Six months of experience in a long term care or skilled nursing environment preferred
  • Minimum of one year of supervisory or leadership experience in a healthcare setting preferred
