Job summary

Salem of Cascadia LLC is seeking a compassionate LPN Resident Care Manager in Oregon to oversee resident care planning, coordinate clinical services, and support an interdisciplinary team.

You will monitor resident conditions, ensure high-quality, person-centered care, and mentor nursing staff to uphold regulatory standards and effective communication with residents and families.

Qualifications

  • Graduate of an accredited practical nursing program.
  • Current and active LPN license in the State of Oregon.
  • CPR certification valid in Oregon.
  • Experience in long term care or skilled nursing preferred.
  • Supervisory or leadership experience preferred.

Responsibilities

  • Oversee and coordinate resident care planning and clinical services.
  • Conduct resident assessments and assist in developing individualized care plans.
  • Monitor resident conditions and communicate changes with physicians and interdisciplinary team members.
  • Review physician orders, test results, and treatment plans to ensure continuity of care.
  • Utilize electronic medical records to document and maintain accurate clinical information.
  • Communicate with residents and families regarding care plans, concerns, and updates.
  • Participate in clinical meetings and collaborate with nursing leadership and other departments.
  • Provide education and support to residents, families, and staff regarding care needs and treatment plans.
  • Support admissions, transitions of care, and discharge planning processes.
  • Ensure clinical documentation is accurate, complete, and compliant with regulations and facility standards.
  • Guide nursing staff to promote a positive, collaborative team environment.

Join our nursing leadership team as an LPN Resident Care Manager and earn up to $8,000 in additional bonus compensation throughout your first year with us.

The bonus is paid as you reach the following milestones:

  • 90 Days: $1,500
  • 6 Months: $1,500
  • 9 Months: $2,000
  • 12 Months: $3,000

Total First-Year Bonus Opportunity: $8,000

What We’re Looking For
  • Current LPN license in the State of Oregon
  • Valid CPR certification

Requirements
Education
  • Graduate of an accredited practical nursing program
Licenses/Certifications
  • Current and active LPN license in the State of Oregon
  • Valid CPR certification
Experience
  • Six months of experience in a long term care or skilled nursing environment preferred
  • Minimum of one year of supervisory or leadership experience in a healthcare setting preferred
