Frontier Senior Living in Spokane Valley, WA, seeks an outstanding Resident Care Coordinator to ensure quality care for residents. Active HCA or CNA in WA is required; CPR certification is essential.

You will train caregivers and coordinate care plans with families and community leadership. Responsibilities include managing daily tasks, scheduling service plans per state guidelines, and coordinating with home health and hospice providers.

• Experience supervising staff in a senior care setting preferred.
• Current CPR certification required.

Responsabilidades

• Coordinate care plan conferences with residents and families.
• Maintain caregiver task lists and update ADL changes.
• Schedule service plan dates per state guidelines.
• Order supplies and durable medical equipment as needed.
• Coordinate with home health, hospice, and ancillary providers.
• Complete MAR audits as directed by RN.
• Assist RN with incident tracking and investigations.
• Support recruitment, training and coaching of staff.

The Resident Care Coordinator requires an understanding and commitment to the principals and values of the community as well as an ability and willingness to train caregivers in tasks required for delivering quality care to the residents of the community.Primary Duties and Responsibilities:Conducts Coordination of Care plan conferences with each Resident and Responsible family membersMaintains current tasks lists for caregivers, and updates timely as ADL changes occurSchedules the service plan dates according to state regulation guidelinesOrders supplies as well as durable medical equipment as neededAssure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.Completes MAR audits as recommended by RN.Monitors the Medication Room to assure organization and cleanliness.Assists RN with tracking and recording incident reports.
