Job summary

Avamere Communities in Portland, OR is seeking a Resident Care Coordinator to oversee resident care needs and staffing, ensure proper documentation, and support the Director of Health Services. You will train Caregivers and oversee daily operations to ensure compliance with policies.

The RCC collaborates with staff to maintain safety, quality care, and a warm environment for residents; weekend/on-call coverage is required.

Qualifications

• Requires high school diploma or equivalent.
• CPR/First Aid certification active.
• Experience as Caregiver or Medication Tech preferred.
• Able to obtain Food Handler’s Card.

Responsibilities

• Coordinate resident care needs and staffing.
• Document changes in residents’ status and appointments.
• Provide orientation and on-the-job training to Caregivers and Med Techs.
• Assist with daily living activities and resident care.
• Manage scheduling of Caregiver and Med Tech staff.
• Maintain documentation of all care plans and notes.
• Participate in training and quality assurance meetings.
