Job summary

Prestige Senior Living – Southern Hills in Salem, OR is seeking a Resident Care Coordinator to support resident care by coordinating health services and guiding the care team. The role requires direct care duties as needed and collaboration with leadership to ensure compliant staffing and service plans.

Ideal candidates have experience in assisted living or health services, a high school diploma or GED, and the ability to work weekends/holidays.

Qualifications

• Detail oriented and able to handle challenges while caring for residents.
• Experience in assisted living or health services is preferred.
• Ability to work on-call, weekends and holidays as scheduled.

Responsibilities

• Partner with health services leadership in developing and implementing Cooperative Service Plans.
• Coordinate and assist in interviewing, hiring, onboarding and training health services staff.
• Collaborate with leadership to maintain health services scheduling to cover days off while meeting resident care needs and state requirements.
• Maintain medication carts and rooms in organized, clean, and safe condition; submit orders as prescribed.
• Coordinate information on residents' condition changes and communicate to appropriate individuals.
• Work as a Medication Aide and/or Personal Care Attendant as needed to cover shifts and stay informed on resident care concerns.
• Educate, monitor and practice safety procedures relating to equipment, chemicals, tools and infection control.
