Job summary

Burlington Health and Rehab Center is seeking a compassionate Resident Care Coordinator to welcome residents and families from admission through discharge. The role requires strong communication, organization, and multitasking to ensure a smooth experience for residents in a busy facility.

The position is full-time with a Monday–Friday 8:00 AM–4:00 PM schedule and occasional weekend MOD shifts. Pay is based on experience and qualifications, with benefits typically offered by the facility.

Qualifications

• High school diploma required; an associate degree is preferred.
• Experience in healthcare, hospitality, or customer service is desirable.
• Strong communication and interpersonal skills are essential.
• Highly organized with attention to detail and ability to multitask.

Responsibilities

• Ensure open rooms are clean, staged, and move-in ready.
• Coordinate with Admissions, Marketing, and Social Services for admissions and discharges.
• Conduct rounds to address resident needs and requests.
• Maintain clean, welcoming common areas and coordinate post-discharge turnover.
• Conduct follow-up calls with families (Caring Partner calls) and greet new residents upon admission.
• Assist with admission documentation and orientation to services and amenities.
• Coordinate discharge readiness and timing; conduct a brief resident satisfaction survey.
