Job summary

HIGHLANDHILLS seeks an experienced care coordination professional to work with residents and families, guiding plans of care and treatment discussions. You will participate in rounds, answer questions, and ensure timely charting and documentation.

Responsibilities include coordinating admissions/discharges, referrals, and utilization reviews, while collaborating with physicians, payers, and care teams to maintain cost-effective, high-quality care.

Qualifications

• Bachelor's Degree in Nursing or Social Work is required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare/Medicaid programs and benefits.

Responsibilities

• Communicate with residents and families about care plans and status.
• Attend rounds/stand-ups to coordinate care with the team.
• Complete documentation accurately and timely.
• Coordinate admissions, discharges, referrals, and utilization reviews.
• Promote cooperation among residents, families, staff, and physicians.
