Job description

$22-$24 per hour (DOE)

Full-time, Day Shift, weekends included.

JOB SUMMARY:

A Resident Care Coordinator works closely with the Health Services team at our Community Based Care facility to ensure quality care for all residents. This role involves organizing and assisting with training new care staff and providing ongoing mentorship to current employees.

RESPONSIBILITIES:

• Coordinate onboarding and orientation for new employees
• Act as lead trainer for the care team (including Caregivers and Med Techs)
• Oversee ongoing training for all staff
• Review assignments with the Executive Director and Director of Health Services
• Assist residents with activities of daily living, encouraging self-care and independence
• Support leadership in creating staff schedules
• Document any resident change of condition
• Willing to cover shifts when needed
• Perform other duties as assigned

REQUIREMENTS:

• Must be at least 18 years old

Achieved a deficiency-free DHS survey in 2019 and 2022!
