Job summary

Eden Vista Greendale, part of the Vista Senior Living Management family, is seeking a Resident Care Coordinator for first shift. You will support residents with compassionate, family‑like care and ensure accurate medical records and timely communications with nursing staff.

Ideal candidates bring at least one year in a health care facility, a leadership background, and a commitment to resident‑centered care and continuous quality improvement. A GED/HS diploma is required.

Qualifications

• High school diploma or GED.
• At least one year of experience in a health care facility.
• One year prior leadership experience.

Responsibilities

• Maintain a positive, professional demeanor toward residents, visitors, families, and co-workers.
• Provide clear clinical handoffs to nursing staff for seamless admission care.
• Maintain residents' charts and ensure up-to-date documentation in point click care.
• Keep medical records in line with state regulations as directed by Wellness leadership.
• Complete incident reports as needed and monitor MAR/PCC orders daily.
• Assist with resident/employee testing (e.g., COVID) when required.
• Coordinate appointments and transportation for residents.
• Send physician orders for monthly review and perform monthly medication cart audits.

Position: Resident Care Coordinator: 1st shift

Essential Functions:

• Maintains a positive and professional demeanor toward residents, visitors, families, and co‑workers.
• Provide clear clinical handoffs to nursing staff to ensure seamless care upon admission.
• Maintains all residents’ charts and ensures they are complete and up to date. Assist DOW with developing chart for resident within point click care.
• Maintains all medical records according to state regulations as directed by the Director of Wellness.'
• Completes incident reports as needed.
• Checks Medication Administration Record and makes out requisitions from the lab book daily. Enter orders into MAR/PCC as needed. Follow up on medications not in facility to ensure they are ordered. (includes diabetic supplies)
• Assist with testing of residents/employees as needed, such as COVID testing.
• Maintains confidentiality of resident information.
• Coordinate appointments and transportation for residents.
• Send Physician orders for review monthly.
• Performs medication cart audits on a minimum, monthly basis.

Qualifications/Skills/Educational Requirements

• High school diploma or general education degree (GED).

Experience

• At least one year of experience in a health care facility and knowledge of medical terminology.
• One (1) year prior experience in a leadership role.
• Commitment to resident-centered care and continuous quality improvement.
