Job summary

Peregrine Senior Living in Syracuse, NY is seeking an energetic Resident Care Coordinator to oversee medication management, supervise care staff, and ensure compliance with policies. LPNs are welcome but not required, with opportunities for training and career growth.

Qualifications

• LPN welcomed but not required.
• Bachelor’s in human services with 1 year related experience.
• Associate degree in human services with 3 years related experience.
• High school diploma with 5 years related experience.
• Experience in all aspects of senior living preferred.

Responsibilities

• Oversee medication management program, including chart and medication audits for QA and regulatory compliance.
• Collaborate with physicians to maintain accurate and up-to-date medication orders.
• Support training and orientation for care staff to ensure proper procedures and best practices.
• Ensure adherence to policies and procedures (P&P) with focus on documentation.
• Supervise and schedule resident care department staff per PSL policies and standards.
• Participate in recruiting, screening, and interviewing staff; fill in during shortages.
• Administer medications as allowed per state guidelines when required.

Education

LPN welcome

Bachelor’s in human services + 1 year experience

Associate degree in human services + 3 years

High school diploma + 5 years related experience

Senior living experience preferred

Salary: $62,354- $67,400 – based on experience

Resident Care Coordinator Responsibilities include:

• Oversee medication management program, including chart and medication audits to ensure quality assurance and regulatory compliance
• Collaborate with physicians to maintain accurate and up-to-date medication orders
• Support training and orientation for all care staff to ensure proper procedures and best practices in the care department
• Ensure adherence to company policies and procedures (P&P) in all aspects of care with a focus on documentation
• Supervise, develop, and schedule all resident care department staff providing day-to-day care of the residents according to PSL policies, procedures, and standards
• Serve as a team member in recruiting, screening, and interviewing of all resident care department staff while filling in for staff during unexpected shortages
• Administer medications as allowed per state-specific and PSL guidelines when required.

Resident Care Coordinator Education & Experience:

• Licensed Practical Nurse (LPN), welcomed but not required
• Bachelor’s Degree with major course work in human services and one year of full-time work experience in a related field or
• Associates degree with major course work in human services and three years of full-time work experience in a related field or
• High school diploma or equivalency plus 5 years related experience required.
• Experience and knowledge in all aspects of senior living, preferred
