Job summary

The Bluefields is seeking a dedicated Resident Care Coordinator to support residents' health and well-being by coordinating care services and managing care plans.

You will work with the Executive Director, nurses, and care staff to ensure a safe, person-centered environment, while maintaining regulatory compliance and high care standards. Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.

Responsibilities

• Develop and manage individualized service plans (ISPs) with clinical staff, residents, and families.
• Supervise caregiving staff and support scheduling for adequate staffing.
• Engage residents and families with care-related communications and meetings.
• Ensure compliance with state regulations, company policies, and industry best practices.
• Coordinate with nursing, physicians, therapists, and leadership.
