Job summary

Transforming Age’s AlmaVia of San Rafael is seeking a dedicated Resident Care Coordinator to support the care team in a memory care and assisted living environment. You will assist in screening and orienting new staff, coordinate medication refills, and ensure compliance with care plans and safety protocols.

You must have a high school diploma, with six months in long-term care; knowledge of Title 22 regulations and proficiency in Microsoft Office are essential.

Qualifications

• High School Diploma or equivalent; some college coursework or an Associate/Bachelor's preferred.
• At least six months of experience in a long-term care or related setting.
• Knowledge of RCFE medication programs and Title 22 regulations.
• Proficient in Microsoft Office, especially Word, Excel, and email.
• Strong written and verbal communication and collaboration with diverse teams.

Responsibilities

• Assist in screening and orienting new Resident Assistants and Medication Technicians.
• Conduct audits and ensure follow-up on identified items.
• Manage medication refill processes and timely availability for residents.
• Coordinate and orient new residents during move-in and onboarding.
• Support resident care plans and safety protocols.
