Job summary

Hood River Post Acute is seeking a caring Nursing Assistant to support residents in daily activities and care. You will report changes in condition to the Nurse Supervisor and document information clearly on care records.

Maintain a warm, safe environment and help with mobility, hygiene and meals. Responsibilities include preparing rooms, greeting residents, assisting with transfers, and following facility policies.

Responsibilities

• Report changes in resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
• Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary).
• Ensure that the resident’s room is ready for receiving the resident (bed made, name tags up, admission kit available).
• Greet residents and escort them to their room; introduce residents to roommates and staff as appropriate.
• Assist with feeding and personal care, and check residents regularly to meet their needs.
• Observe and report pressure areas or skin breakdowns to prevent decubitus ulcers.
• Maintain confidentiality of resident care information and report suspected disclosures.
• Participate in nursing reports and follow established policies and procedures.
